To enrol, please complete the form, using the timetable and class age list above, to determine classes. Please use a seperate form for each child.

I understand and agree that my child may be photographed or filmed during a Dancehouse class, rehearsal, eisteddfod competition or Dancehouse performance by Dancehouse staff or their agents. I further consent that my child's image may be used in advertising material, print media, on the Dancehouse website, Dancehouse Facebook page, in the Dancehouse Private Facebook group or on the Dancehouse Instagram account. No names will be used without further permission.* I also agree that my child may be photographed or filmed during a Dancehouse class during an Observation Week, by Parents or Grandparents of other children in the class. I also agree I WILL NOT post photos or video footage taken by me or any family member during the Dancehouse Observation Week classes on Facebook, Youtube, Instagram or other Social Media, if there are any children, than my own in the photos or video.* I have read and understood the Dancehouse Policies (see the Policies page) and I agree to pay the tuition fees for my child's enrolled classes, as they become due. I understand my child's enrolment will remain current until I cancel it in writing, giving at least 14 days notice.
